On your reminder email the subject line is followed by the date the reminder was sent.

Eg

Your upcoming appointment

Aug 9,2014

Every time I see that I think “oh no I missed my appointment””. Only much later in the message does the actual appointment date show up. ( say Aug 15 ) You might want to rethink how the info is presented.
